Fees

My fee is $125 for a one hour individual session and $150 for family or couples counseling. I am an out-of-network provider with Blue Cross/Blue Shield and several other health insurance providers, which means that my services are covered after your annual out-of-network deductible is met. I will provide a monthly statement for services rendered that you can submit to your insurance company for reimbursement.

Getting Started

If we decide to work together you may wish to read, download, and sign the Consent to Treatment form that follows and complete the Intake Form. We can discuss these in our first session and I will answer any of your questions.

The Consent to Treatment forms briefly explain the nature of therapy, limits to confidentiality, and my policies and procedures. Your signing it indicates your consent to treatment.

The Intake Form provides me with basic background information so we can spend the first session talking about what's bothering you rather than you answering a lot of questions. If possible, please download, complete, and bring to your first session.

The Monthly Health Assessment lets me know how you have been doing physically and emotionally over the last 4 weeks.
